To help those who are unable to complete the registration, family members, care givers, and other companions may register on behalf of another eligible person. Those who do not have internet access or someone to complete the registration on his/her behalf may call (319) 291-2413 to register.
Due to a high demand for vaccines, and a limited supply, there are a limited number of appointments available for eligible persons.
After you complete the registration form you will receive a cancellation code that will be required for appointment cancellation.
If you schedule an appointment, but upon Health Department review do not meet the eligibility criteria noted in the screening questions during the registration process, your appointment will be cancelled.
***ATTENTION*** Before vaccine can be administered, consent must be obtained. On the day of the event, a paper consent will be available for registrant to review and sign prior to vaccine administration.
